Our Philosophy

Pillars before pathology.

Modern psychiatry is quick to name what is wrong and slow to ask what is missing. Before we reach for a diagnosis, we look at the five foundations that quietly hold a person up — or slowly wear them down. This is the framework Dr. Raena Khorram built her practice around, and it shapes every evaluation at Bryn Mawr Mental Health and Wellness.

The check-in lives on The Pillars Project™, Dr. Khorram's clinical framework site. Ten questions. No sign-up. Results in minutes.

A label is not always the answer.

If you're feeling anxious, unfocused, exhausted, or simply "not yourself," it's tempting to jump straight to a diagnosis. ADHD. Depression. Perimenopause. Burnout.

But often, the body is asking for something quieter first — the rebuilding of foundations that modern life slowly erodes. Sleep. Rest. Nourishment. Movement. Connection.

Pillars Before Pathology helps us pause, look inward, and see what's actually missing — so any diagnosis or treatment that follows can work better, not harder.

The Five Foundations

Five pillars. One lens.

01

Sleep

The cognitive reset.

Before we call it depression, ADHD, or anxiety, we ask what your sleep actually looks like. Fragmented, delayed, or shortened sleep mimics — and worsens — nearly every psychiatric symptom. Restoring sleep architecture is often the fastest, most under-prescribed intervention in mental health.

02

Rest

Active nervous-system recovery.

Rest is not sleep. It is the deliberate down-shift of a nervous system that has been running on sympathetic overdrive. For high-functioning adults, chronic activation looks like burnout, irritability, and the sense of running on fumes — long before it looks like a diagnosis.

03

Nourishment

The biological building blocks.

The brain runs on what you eat, drink, and absorb. Iron, B12, vitamin D, protein, and omega-3s show up repeatedly in patients told they have treatment-resistant depression. We look at the biology before we escalate the medication.

04

Movement

Somatic emotional release.

Movement is the most evidence-based antidepressant we have that doesn't require a prescription. It regulates mood, sharpens cognition, and discharges stored stress. Not every patient needs an hour at the gym — but every patient needs a body that moves.

05

Connection

The neurobiology of safety.

Human beings co-regulate. A nervous system that feels safe in the presence of others heals faster than one that doesn't. Loneliness, over-scheduling, and the quiet erosion of intimate relationships are among the most powerful drivers of what we call mental illness.

How this shapes your care.

Every Bryn Mawr Mental Health and Wellness evaluation begins with the pillars. That is why our first appointment is 90 minutes, not 15 — there is no way to see the foundation of a life in a rushed visit.

When medication is right, we prescribe it. When therapy is right, we deliver it. When accelerated TMS is the answer, we offer it. But we do not skip the step of asking what a person is actually running on.

The Pillars Project™

A companion framework, built by our founder.

Dr. Khorram created The Pillars Project as a free, public-facing tool that puts this framework into anyone's hands — patient or not. Take the two-minute check-in to see which of your pillars may quietly be holding you back, then bring the results into your first conversation with our team.
