What high-demand professionals come to us for
High-functioning depression
Still delivering at a high level while joyless, flat, and running on willpower.
Anxiety and chronic vigilance
Anticipatory dread before board meetings, raises, or earnings — and no off switch.
Sleep that never fully recovers
Early waking, 3 a.m. rumination, and travel that keeps the nervous system on alert.
Adult ADHD recognized late
Careers built on compensation strategies that stop working as scope grows.
Alcohol used as a shutdown mechanism
The nightly wind-down drink that is quietly increasing, addressed without judgment.
Burnout and role transition
Exits, successions, and identity change after decades of being the one who decides.
Professional scrutiny and isolation
Physicians, attorneys, and licensed professionals who are the responsible party all day — and have nowhere to be the patient.

Frequently asked questions
Why out-of-network rather than insurance?+
Insurance panels require diagnostic coding, utilization review, and disclosure of your treatment record to a third party. Working outside of insurance keeps your record between you and your clinician, keeps caseloads small, and lets the length and frequency of appointments be set clinically rather than administratively. Superbills are provided if you choose to seek reimbursement.
How is my privacy protected?+
Appointments are scheduled to minimize waiting-room overlap, notes are kept to the clinical minimum required, and nothing is released to an employer, insurer, or family member without your written authorization. Virtual sessions are conducted on a HIPAA-compliant platform.
I travel constantly. Can care follow me?+
Yes, within licensure. Dr. Khorram is licensed in Pennsylvania and New York, so you can be seen in person in Bryn Mawr and virtually while you are elsewhere in either state.
Are appointments available outside the workday?+
Early morning and later-afternoon slots are held for patients with rigid meeting schedules. Because caseloads are deliberately small, rescheduling is realistic rather than a six-week wait.
Is this therapy, medication, or executive coaching?+
It is clinical care — psychiatric evaluation, expert psychopharmacological management, and 50-minute therapy — not coaching. Coaching does not diagnose or treat depression, anxiety, ADHD, or substance use, and it is not a substitute for a psychiatrist.
What kinds of executives do you see?+
Our executive patients include founders, C-suite leaders, partners, and high-demand professionals in pharma, medicine, law, finance, technology, and consulting. Many are physicians, attorneys, and licensed professionals who carry the added burden of public scrutiny, regulatory exposure, or the expectation that they are always the responsible party in the room.
What does the first appointment involve?+
An unhurried 85-minute psychiatric evaluation covering history, sleep, substances, medical contributors, and the demands of your actual role — followed by a plan you understand before you leave. Follow-ups are 30 minutes.
